If the server does not output an error on crash it is almost always the map's fault.
So far that has always been the case. We have so many maps running and available, we can afford being a bit more strict about maps.
By the way, if a map has a problem, they will always remain listed. Just with a no entry sign in front of them (meaning they will not be selectable in the game panel), that is until they are sorted

And to answer your question, not necessarily no. A map can be a problem once only. Take for example dm_killbox_bunker_xmas :
Has over 2048 Edicts and WILL cause the server to crash with too many players (that happened twice over xmas when we had up to 24 players online)
Ok, this is not often, but if many maps do it not often, then we have a server that crashes often.

Also as I have said somewhere, before. Steam pushes updates (however rare) and these causes maps to simply not work anymore. A good example here are the Junkcity maps which Zen very kindly fixed, but then after realized it was a bad idea when he kept on being killed by mines

I don’t know if it could have been damaged as a result of decompilation. If this was the case initially, it is easier to create a new map.

Just buggy most probably because of antiquated items I would think.....
A quick look in entspy might give a clue?
